Prisma Health Heart & Vascular Institute - Advanced Practice Provider- Heart Failure

Upstate Market, Greenville, SC

Position:

The Heart Failure Advance Practice Provider (APP) delivers comprehensive, patient-centered care to individuals with acute and chronic heart failure in both the inpatient and outpatient settings. In this collaborative role, you will work closely with cardiologists, pharmacists, and nurses to assess, plan, and implement comprehensive patient care. This position requires strong competencies in clinical practice, communication skills, and patient-centered decision-making. 

Responsibilities  

•        Perform comprehensive histories, physical examinations, and ongoing clinical assessments 
•        Develop, implement, and adjust individualized treatments plans using guideline-directed medical therapy (GDMT) 
•        Order, interpret, and act upon diagnostic studies including labs, imaging, and cardiac testing  
•        Educate patients and families on heart failure self-management and lifestyle modification 
•        Participate in specialty clinics (i.e. advanced heart failure, pulmonary hypertension, cardiac amyloid, and hypertrophic cardiomyopathy clinics) 
•        Manage mechanical circulatory support including intra-aortic balloon pumps, Impella assist devices, and extracorporeal membrane oxygenation 
•        Manage remote patient monitoring and other heart failure devices 
•        Advance research through clinical trials 
•        Document care efficiently and accurately in the electronic medical record 

Supervisory/Management Responsibility 

This is a non-management job that will report to a supervisor, manager, director, or executive. 

Qualifications 

•        Graduate of an accredited Physician Assistant (PA) program or Acute Care  
•         Nurse practitioner  (NP) program
•        Current licensure (or eligibility for licensure) as a PA/NP in South Carolina  
•        National board certification  
•        DEA license and prescriptive authority  
•        BLS and ACLS certified  

Work Shift 

•        Monday – Friday 
•        No call, nights, weekends, or holidays
