Description
We are seeking a motivated and compassionate Nurse Practitioner or Physician Assistant to join our multidisciplinary Cystic Fibrosis (CF) program. This full-time role offers the opportunity to care for a complex and unique patient population within a collaborative, team-based environment focused on high-quality, evidence-based care.Position Overview
The APP will play a key role in the longitudinal management of adult patients with cystic fibrosis. This will be primarily in clinic, as well as providing a bridge between outpatient and inpatient teams when patients are admitted. The position emphasizes continuity of care, patient and family education, and close collaboration with pulmonologists and an established CF care team.
Key Responsibilities
- Provide comprehensive care for patients with cystic fibrosis, including assessment, diagnosis, treatment planning, and follow-up
- Attend multidisciplinary CF clinic in Lebanon and Manchester
- Manchester clinics are approximately 2 times per month
- Triage sick calls and see add-on patients when needed
- Help manage disease progression and acute exacerbations in collaboration with CF pulmonologists
- Diagnose and screen for CF complications such as diabetes, bone disease, liver disease, etc., and develop a plan of care based on clinical practice guidelines
- Prescribe and monitor medications, including eligibility and safety monitoring for CFTR modulators
- Collaborate with a multidisciplinary team including nursing, respiratory therapy, nutrition, pharmacy, social work, and behavioral health
- Educate patients and families on disease management, medications, airway clearance techniques, and lifestyle considerations
- Participate in quality improvement initiatives, patient education efforts, and CF program development
- Attend regional and national Cystic Fibrosis conferences along with the team
- Maintain accurate and timely medical documentation
- May support other adult pulmonology programs as well.
Qualifications
- Graduate of an accredited Nurse Practitioner or Physician Assistant program and NCCPA certification (PA) or national NP certification
- Ability to obtain/maintain licensure in New Hampshire and Vermont
- Prior experience in pulmonology, cystic fibrosis, or complex chronic disease management preferred (new graduates with strong interest will be considered)
- Strong clinical judgment, communication skills, and commitment to patient-centered care
- Interest in working within a specialized, team-based care model
